Registered Nurse - Home Health

Job Summary: Join our team as a Registered Nurse specializing in Home Health, where you will play a pivotal role in delivering exceptional healthcare services to patients within the comfort of their homes. Duties: - Perform comprehensive health assessments for patients in a home environment - Create individualized care plans tailored to meet the unique needs of each patient - Administer medications, provide wound care, and deliver prescribed treatments effectively - Monitor patient progress, assess outcomes, and adjust care plans as necessary - Educate patients and families on disease management, medication administration, and self-care practices - Coordinate with interdisciplinary healthcare teams to ensure holistic patient care - Maintain accurate and timely documentation of patient interactions, interventions, and progress

Requirements: - Active Registered Nurse (RN) license in the state of practice - Minimum of 2 years of clinical nursing experience, preferably in home health or related field - Proficiency in ER procedures, anatomy knowledge, catheterization, spinal tap techniques, discharge planning, physical examinations, pain management, ophthalmology assessments - Familiarity with Alora or similar electronic medical record systems - Experience in medical-surgical settings is advantageous
