Job Summary

We are looking for a compassionate and experienced Medical Social Worker to provide essential social work services to our home health patients. As a Medical Social Worker, you will assess patients' social and emotional needs, develop personalized plans of care, and connect them with community resources to support their recovery. You will work closely with our interdisciplinary team to ensure seamless care coordination and discharge planning. Key Responsibilities

  • Patient Assessment: Evaluate patients' social and emotional states, identifying their needs for care and response to treatment.
  • Plan of Care Development: Develop and implement personalized plans of care that address patients' emotional and social needs.
  • Community Resource Connection: Connect patients with community resources, such as housing, shelter, funeral/memorial service arrangements, legal services, and state/federal financial and medication programs.
  • Discharge Planning: Participate in discharge planning activities to ensure smooth transitions to post-acute care settings or community-based services.
  • Interdisciplinary Collaboration: Work closely with physicians, nurses, and other healthcare professionals to ensure comprehensive care coordination.
  • Social Work Assistant Supervision: Supervise, instruct, and evaluate the performance of Social Work Assistants (BSW) to ensure compliance with company, government, and professional standards.
  • Documentation and Reporting: Maintain accurate and timely records of patient assessments, plans of care, and progress notes.

Requirements and Qualifications

Essential Qualifications:

  • Education: Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education.
  • Licensure: Social Worker licensure in the state of practice if required by state law or regulation.
  • Experience: Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice.
  • Skills: Excellent oral and written communication and interpersonal skills. Ability to read, write, and speak fluent English.

Preferred Qualifications:

  • Driver's License and Transportation: Valid driver's license, auto insurance, and reliable transportation.
  • CPR Certification: Proof of current CPR certification.
  • Knowledge of Medications: Knowledge of medications and their correct administration.
  • Organizational Skills: Ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
