Pharmacy Technician - Remote Patient Monitoring (RPM) Program Health Coach and Data Entry Technician

Job Description: We are seeking a dynamic and motivated Pharmacy Technician to join our team as our RPM Program Health Coach and Data Entry Technician at our corporate office. In this multifaceted role, you will be responsible for providing personalized coaching and support to patients enrolled in our Remote Patient Monitoring program and also assisting our other pharmacy locations by performing prescription data entry. Your primary duties will include contacting patients to facilitate their onboarding process, educating them on the proper use of blood pressure monitors, and monitoring their adherence to monitor usage guidelines. Additionally, you will oversee the logistical aspects of the RPM program, including managing inventory, packaging, shipping of monitoring devices to patients and marketing related activities. As a key member of our team, you will play a crucial role in promoting patient engagement, adherence, and overall wellness through proactive outreach and support. Interactions: This position reports to the Director of Clinical Services/RPM Director. This role will also require collaboration with other Shrivers pharmacists and pharmacy interns. Key Responsibilities:

  • Conduct outreach to patients to initiate their enrollment in the Remote Patient Monitoring

program.

  • Provide comprehensive education and training to patients on the proper use of blood pressure

monitors and other monitoring devices.

  • Monitor patient adherence to prescribed monitoring protocols and intervene or triage as

needed to address any challenges or concerns.

  • Maintain accurate records of patient interactions, monitor usage, and program outcomes.
  • Collaborate with healthcare providers and other team members to ensure seamless

coordination of care and support for patients.

  • Manage inventory of monitoring devices and supplies, including ordering, tracking, and

restocking as needed.

  • Prepare monitoring devices for shipment to patients, ensuring timely and accurate delivery.
  • Participate in ongoing training and professional development activities to stay current on best

practices and emerging trends in remote patient monitoring and healthcare technology.

  • Contribute to the development and implementation of strategies to enhance patient

engagement, satisfaction, and outcomes within the RPM program.

  • Remote prescription data entry from corporate office to assist the workflow of all our

pharmacies and any special projects requiring assistance Qualifications:

  • High school diploma or equivalent required. Registered pharmacy technician required;

certification as a Pharmacy Technician preferred.

  • Prior experience working as a pharmacy technician or in a related healthcare setting (ie medic,

RN) is strongly preferred.

  • Strong communication skills, with the ability to effectively educate and engage patients via

phone or other remote communication channels.

  • Knowledge of basic medical terminology and familiarity with common chronic conditions, such

as hypertension, preferred.

  • Excellent organizational skills and attention to detail, with the ability to manage multiple tasks

and priorities in a fast-paced environment.

  • Proficiency with computer applications and electronic health record systems.
  • Commitment to providing compassionate, patient-centered care and promoting positive health

outcomes.

  • Ability to work independently and collaboratively as part of a multidisciplinary team.
  • Flexibility to adapt to evolving job responsibilities and program requirements.
