We are looking for a full-time (FT) RN specializing in cardiology and heart failure to remotely monitor and manage patients with heart failure. Bilingual candidates (English and Spanish speaking) are highly preferred. In this role, the RN will track and trend vital signs, virtually assess patients, document and work with the clinical care team to coordinate care. Our RNs will use their excellent clinical and interpersonal skills to work with patients and the members of the clinical care team to provide high standards of clinical care. They will think critically and creatively about what is needed to enhance the patient's care. The ideal candidate has strong clinical judgment, critical thinking, and interpersonal skills to provide high-quality patient-centered care. They will proactively assess trends in patient data, identify potential concerns, and intervene appropriately to optimize heart failure management. Additionally, they will play a key role in patient education, empowering individuals and their caregivers to manage symptoms effectively and adhere to their care plans. We are seeking full-time coverage Monday through Friday from 8:00 AM to 4:30 PM PST. What you’ll be doing:
