Make a Difference The Manager of Case Management provides onsite leadership to ensure daily department operations function effectively and efficiently to meet the needs of all customer and regulatory groups. You will act as a consultant to ensure the severity of illness and intensity of services interval criteria evaluations are completed on all patients as designated by Case Management goals. You will ensure that case managers assess, plan, and develop plans for patients based on recognized standards of practice for case management. You will support a culture of continuous quality improvement. 

Exceptional Skills and Qualifications Applicants for this position should be able to demonstrate competency as a Case Manager, exhibit team-building skills, and effect change by assuming leadership roles in interdisciplinary teams.

  • 2 year / Associate Degree RN: Graduate of an accredited school of nursing or Master’s degree in social work (Required)
  • Bachelor of Science in Nursing preferred.
  • Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC) or licensed as a Social Worker either LSW or LCSW in the state of Indiana (Required).
  • 3-years of clinical experience in a hospital setting is required.
  • 2-years management experience preferred.
  • Case management experience preferred. 
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM) or equivalent certification within 3 years from hire date (Required).
  • Understands the relationship of individual patients, units, and/or departments to the network, to facilitate achievement of optimal goals with those relationships.
