Job Summary: We are seeking a detail-oriented and proactive Clinical Intake Nurse to join our team. This remote role is responsible for managing incoming referrals, coordinating with hospital partners, and supporting the development and maintenance of a strong and balanced patient census.
Key Responsibilities
Monitor hospital referral portals daily to identify and respond to new patient referrals in a timely manner
Communicate effectively with hospital case managers and social workers to coordinate patient admissions
Review clinical information to ensure appropriate patient placement and eligibility
Support and maintain a healthy census mix aligned with organizational goals
Collaborate with internal teams to facilitate smooth admission processes
Maintain accurate and up-to-date documentation of all referral and intake activities
Qualifications
Experience with clinical intake, case management, or admissions preferred
Strong communication and relationship-building skills
Ability to work independently in a remote environment
Proficiency with electronic medical records and referral portal systems
