[Hiring] Family Nurse Practitioner Enhanced Primary Care @Springbrook

Role Description The Article 28 Primary Care Clinic is expanding its services and seeking a qualified, compassionate Family Nurse Practitioner (FNP) to join our interdisciplinary healthcare team. The ideal candidate will be committed to improving access to high-quality, person-centered healthcare for individuals with intellectual and developmental disabilities (IDD). This role provides the opportunity to deliver holistic primary care within a clinic designed to meet the unique needs of this specialized population.

• Provide comprehensive primary and preventive care across the lifespan within an Article 28 framework.
• Conduct assessments, diagnose conditions, and develop evidence-based treatment plans.
• Collaborate with physicians, providers, and interdisciplinary teams to coordinate individualized care.
• Support health promotion, patient education, and chronic disease management tailored to individuals with IDD.
• Participate in quality improvement initiatives and contribute to the enhancement of clinic workflows and clinical protocols, with a focus on health equity and reducing barriers to care.
• Provide clinical leadership and mentorship to nursing and support staff, fostering a culture of compassion, compliance, and excellence.
• Maintain clinical documentation in accordance with regulatory standards and organizational policies.
• Deliver services both in-person and through telehealth as appropriate.
• Other duties as assigned.

Qualifications

• Current New York State Family Nurse Practitioner license and certification (MSN or DNP required).
• Minimum of 3 years of clinical experience in primary care, family medicine, or internal medicine required.
• Experience with the IDD population is preferred but not required—training and support will be provided.
• Strong clinical assessment, communication, and interdisciplinary collaboration abilities.
• Commitment to providing inclusive, equitable, person-centered, and accessible care for individuals with IDD.
• Willingness to obtain required Intellectual and Developmental Disabilities (IDD)–specific certifications and complete related educational training.
• Duties require professional verbal and written communication skills.
• Meet the requirements to drive Springbrook vehicles as required.

Benefits

• Salary ranges from $120,000.00 to $160,000.00 per year based on experience and education.
