Shared Living Case Manager

Description Position Summary: The Shared Living Case Manager is responsible for developing, coordinating, and monitoring individualized Shared Living placements to ensure the health, safety, and personal goals of individuals served. This role includes skill training, supportive counseling, case management, and crisis intervention, while maintaining compliance with all applicable regulations and quality standards. The Case Manager works closely with individuals, caregivers, families, and multidisciplinary teams to promote a person-centered approach and meaningful community integration. Schedule: Monday - Friday, 8 am - 4 pm

Key Responsibilities:

  • Provide direct skill training, case management, supportive counseling, and crisis intervention as needed.
  • Develop and supervise individual Shared Living placements under the guidance of the Shared Living Assistant Director.
  • Conduct regular home visits to assess safety, well-being, ensure program compliance and satisfaction of individuals served.
  • Coordinate services with the Shared Living Assistant Director and other care providers to ensure integrated and effective support.
  • Attend Service Planning meetings and actively participate in the development, implementation, and review of Individual Service Plans (ISPs).
  • Maintain accurate and timely documentation of all case management activities in compliance with agency and funding requirements.
  • Ensure program compliance with all applicable state funding agencies, certification regulations, and internal quality service standards.
  • Facilitate services that promote a person-centered approach aligned with best clinical practices.
  • Provide training, supervision, and evaluation of Shared Living caregivers as assigned.
  • Collaborate with the Shared Living Director and Caregiver Coordinator in placement processes.
  • Coordinate with healthcare providers, community agencies, and other stakeholders to support individual needs and goals.
  • Provide transportation for appointments, activities, and meetings as needed.
  • Maintain required trainings as assigned
  • Perform other job-related responsibilities as assigned.

Required Qualifications:

  • Minimum of 2 years’ experience in case management, direct support, or a related human services role.
  • Knowledge of person-centered planning, developmental disabilities, and community resources.
  • Strong organizational, documentation, and communication skills.
  • Ability to work independently while maintaining collaborative relationships with teams and caregivers, and community partners.
  • CPR/First Aid Certified Trainer (or ability to obtain certification within designated timeframe).
  • Valid driver’s license, reliable transportation, and willingness to travel extensively.

Preferred Qualifications:

  • Bachelor’s degree in Human Services, Social Work, Psychology, or a related field.
  • Experience in Shared Living, Adult Foster Care, or home-based support models.
  • Experience supporting individuals living independently or in community-based settings.
  • Familiarity with Medicaid, MassHealth, and related funding sources.
  • Knowledge of Electronic Health Record (EHR) systems.
  • Bilingual or multilingual abilities.
