  • Responsibilities
  • Conduct individual and group coaching sessions to promote behavior change, wellness, and chronic condition management. Develop personalized wellness plans with clients, set goals, and provide resources to track progress.
  • Provide onboarding, education, and ongoing support to patients using remote patient monitoring devices (e.g., Bluetooth blood pressure cuffs, glucometers), ensuring accurate data collection and consistent engagement.
  • Monitor RPM dashboards to identify trends, encourage adherence, and proactively reach out to patients who need support with device use, lifestyle habits, or care plan follow-through.
  • Collaborate with healthcare providers to offer chronic condition coaching based on national clinical guidelines.
  • Assist patients in scheduling medical appointments and making necessary internal referrals.
  • Facilitate health promotion programs such as group challenges, educational workshops, and biometric screenings, and work with the client and care team to develop wellness strategies tailored to the population.
  • Participate in client engagement events and contribute to the development of wellness program offerings, coordinating with other vendors (carriers, EAP, etc.) to inform the client population of available resources.

  • Qualifications
  • Bachelor’s degree in health science, counseling, psychology, nutrition, social work or related field.
  • Master’s degree preferred.
  • Certification from the National Board for Health and Wellness Coaches (NBHWC) required.
  • Minimum of 2 years’ experience as a health coach, including conducting one-on-one and group coaching.
  • Experience with digital health tools, remote patient monitoring programs, and/or telehealth platforms preferred.
  • Strong background in health promotion, chronic condition management, and behavior change methodologies.

  • Excellent time management, multitasking, prioritization, and deadline-driven work.

  • Pay and Benefits

Pay Range: $27,000 - $40,000/yr for a 20-hour/week schedule. The actual offer may vary based on geographic location and candidate experience/skill level.
