Pharmacist, West & Central U.S. Coverage
Job Description:
- Perform Medication Therapy Management (MTM) interventions
- Complete medication reconciliation and identify optimization opportunities
- Provide patient-centered education on medications and disease management
- Support chronic care initiatives (e.g., diabetes, hypertension, COPD, CHF)
- Use behavioral techniques such as motivational interviewing to improve adherence
- Collaborate with providers on treatment planning and medication adjustments
- Detect and resolve medication-related risks and discrepancies
- Ensure timely and accurate documentation of all encounters
- Facilitate smooth care transitions and ongoing care coordination
Requirements:
- Doctor of Pharmacy (PharmD)
- Active pharmacist license in at least one of the following states: CA, OR, WA, AZ, NM, NV, UT, ID, MT, WY, KY, OK, AR, LA, WV, NH, VT
- Open to obtaining additional licensure as required
- Board certification (e.g., BCPS, BCGP) preferred
- Experience in: MTM delivery, Ambulatory or outpatient care, Chronic disease management programs, Patient engagement and education
- Familiarity with programs such as CCM, TCM, RPM is beneficial
