Salary: $100,000 - $105,000 per year and benefits Responsibilities: Works closely with members of the health care team in the development of the plan of treatment, including discharge plans, based on the psychosocial assessment, and accepted social work interventions. Performing assessments and interventions as needed for patients. Maintains accurate and timely records including clinical and progress notes according to department policy. Provides crisis intervention for patients, including assessment and treatment of emotional and behavioral problems. Consult and coordinate with family and/or support system regarding client treatment and discharge planning. Qualifications: Must have 3 years of unsupervised, post licensure experience. Master's degree in Social Work or Counseling from an accredited program. Active and unrestricted LICSW licensure in the state.
