Job Description:

HealthConnect Solutions is seeking a dedicated and experienced Registered Nurse Lead to join our Remote Patient Monitoring and Home Telehealth team. As a Remote Patient Monitoring Home Telehealth Care Coordinator, you will play a critical role in providing high-quality care to patients in the comfort of their homes. You will be responsible for managing and coordinating remote patient monitoring services, ensuring the delivery of seamless and efficient care.

Responsibilities:

• Coordinate and oversee remote patient monitoring services for home telehealth patients.
• Monitor patient data and respond to alerts, ensuring timely intervention when needed.
• Collaborate with healthcare providers to develop and implement individualized care plans.
• Provide patient education and support to promote self-management of chronic conditions.
• Maintain accurate and up-to-date patient records in compliance with HIPAA regulations.
• Conduct regular virtual check-ins with patients to assess their health status and address any concerns.
• Serve as the primary point of contact for patients and their families, providing compassionate and effective communication.
• Lead and mentor a team of nursing staff, ensuring high standards of care and professional development.

Requirements:

• Current and unrestricted RN license.
• Minimum of 3 years of nursing experience, preferably in a telehealth or home health setting.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Proficient in using electronic health records (EHR) and telehealth technology.
• Ability to work independently and as part of a team.
• Experience with remote patient monitoring devices and software is a plus.

Educational Qualifications:

• Associate’s or Bachelor’s degree in Nursing required.
• Additional certification in Telehealth or Remote Patient Monitoring is preferred.
