Job Title: HCC Medical Coder - Remote Job Overview We are actively seeking a dedicated and experienced Medical Coder specializing in Risk Adjustment and Hierarchical Condition Category (HCC) coding to join our esteemed team. Key Responsibilities • Accurately review and code medical records and encounters for diagnoses and procedures, adhering to Risk Adjustment and HCC coding standards. • Implement ICD-10-CM, CMS-HCC, and other pertinent coding regulations to ensure compliance and precision. • Ensure the completeness, accuracy, and integrity of coded data. • Address and resolve any discrepancies between clinical documentation and diagnosis coding. • Stay abreast of the latest coding guidelines, regulations, and updates related to Risk Adjustment and HCC coding. • Uphold HIPAA compliance and maintain patient confidentiality and data security. • Collaborate effectively with healthcare providers and team members to clarify documentation and resolve coding inquiries. • Engage in ongoing education and training to enhance coding proficiency and effectiveness. • Generate reports on coding activities, accuracy, and trends. • Support coding audits (both internal and external) to ensure adherence to coding practices. • Identify and recommend enhancements to the coding process and documentation accuracy. Required Skills • Valid CPC or CCS certification (required); CRC or advanced certifications are preferred. • Minimum of 2 years of experience in Risk Adjustment and HCC coding within a healthcare setting. • In-depth understanding of ICD-10-CM coding guidelines and CMS-HCC risk adjustment methodologies. • Proficient with electronic health record (EHR) systems and coding software. • Exceptional attention to detail, strong analytical capabilities, and the ability to work autonomously. • Excellent communication and interpersonal skills for effective collaboration. • Knowledgeable about HIPAA and other compliance regulations. Qualifications • Certification: CPC or CCS (mandatory). • Minimum 2 years of relevant experience in Risk Adjustment and HCC coding in a healthcare environment.
