About the position As an Intake Coordinator LVN/LPN at AccentCare, you will play a crucial role in the healthcare journey of our patients and their families. In this hybrid role, you will work in the office on Fridays and Mondays, while Saturdays and Sundays will be remote. The working hours are from 8 AM to 7 PM, allowing for a balanced work-life schedule. Responsibilities • Coordinate patient intake processes and ensure all necessary documentation is completed accurately. , • Communicate effectively with patients and their families to provide information and support regarding their care. , • Maintain knowledge of state and federal regulations and accreditation requirements related to patient care. , • Perform data entry tasks and manage patient records in compliance with healthcare standards. , • Collaborate with healthcare team members to ensure seamless patient care and service delivery. Requirements • Valid license as a Licensed Vocational Nurse (LVN) or Registered Nurse (RN). , • Minimum of one year experience as an LVN or RN in a medical setting, preferably in home health. , • Knowledge of state and federal regulations and accreditation requirements. , • Strong data entry skills and attention to detail. , • Excellent verbal and written communication skills, particularly over the telephone.
