About the position CVS Health is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join their Primary Care team at the Wilderness Oak clinic in Schertz, Texas. This full-time position involves collaborating with a dedicated clinical care team to provide evidence-based care to patients, focusing on assessment, diagnosis, treatment, and health management. The role emphasizes patient-centered care and effective communication within a team environment, aiming to enhance health outcomes for a diverse patient panel. Responsibilities • Provide patient-centered quality and safety care by assessing, diagnosing, and treating patients' medical needs. , • Perform accurate and complete history and physical assessments of patients. , • Counsel and educate patients and families on health maintenance and respond to inquiries. , • Document all patient care within an electronic health record according to policies and procedures. , • Collaborate with the clinical care team to ensure high-quality, patient-centric care. , • Participate in clinical care team huddles and lead when necessary. , • Communicate effectively with team members and other health professionals. , • Manage clinical and non-clinical duties efficiently and adapt to new patient care models. , • Assist with hiring, development, and evaluation of the clinical care team as appropriate. Requirements • Completion of a master's degree level Family Nurse Practitioner program or equivalent with current National Board Certification and State of Employment license. , • Active Advance Practice Registered Nurse or Nurse Practitioner license in good standing. , • A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner preferred. , • Working knowledge of quality measurement in chronic disease management. , • Effective verbal, written, and electronic communication skills. , • Outstanding organizational skills and ability to multi-task. Nice-to-haves • Willingness to obtain multi-state licensures. , • Experience in family medicine or primary care practice.
