- Open availability to tutor a minimum of 4 - 6 hours per week
- Updated Clearances: FBI, Police Record and Child Abuse (obtained within current year)
- Mandated Reporter Certificate
- Tuberculosis Test Results (TB)
- Completion of Act 24 Arrest or Conviction form
- Completion of Act 168 Form (PA)
- 2-3 Reference Contacts
