Health Coach (Fully Remote)
About the position
The Health Coach at CVS Health plays a vital role in supporting Medicare members through Aetna's Healthy Aging Support Program. This fully remote position focuses on providing health coaching, education, and resources to help members manage chronic conditions and improve their overall health. The Health Coach will conduct group coaching sessions, assist members in navigating their healthcare, and ensure they have access to necessary resources.
Responsibilities
- Educate members on topics such as Managing Medicines, Nutrition, Safety, Benefits/Resources, Mindfulness, and working with Care Teams. ,
- Assist members in engaging with their healthcare providers and Aetna Care Management teams. ,
- Document all member interactions in the appropriate member records. ,
- Demonstrate empathy towards members and their caregivers. ,
- Troubleshoot resource unavailability and secure support to close member gaps. ,
- Follow program workflows and follow up with members to review personal action plans.
Requirements
- At least 2 years of experience as a Health Coach. ,
- Health Coach certification is required. ,
- Availability to support 8:00 AM - 5:00 PM Mountain Time or 7:00 AM - 4:00 PM Pacific Time.
Nice-to-haves
- Community health or public health experience preferred. ,
- Managed Care/Medicare/Medicaid experience preferred. ,
- Bilingual preferred. ,
- Bachelor's degree preferred.
