MLTC Care Manager / MSW or LMSW or LCSW - Hybrid (Manhattan, NY) to Remote $77,000 to $90,000+ with Generous Benefits $15,000 sign on bonus for candidate with NY State LMSW or LCSW Remote job after initial hybrid 4 to 6 months training (onsite 2-3 days per week during training) Training Location: Manhattan, NY Full-time, Permanent Role - Monday through Friday, Business Hours Main goal for Floating Care Manager is to address any urgent issues experienced by members during absence of their assigned Care Manager. Summary: Provide care management coverage to Health Plan members whose primary Care Manager is out of the office. Performs telephonic assessments of members' medical, psychosocial, physical, and spiritual needs and puts in appropriate interventions. Develop, implement, and monitor Person Centered Service Plan, and ensure continuity of care across all healthcare settings by collaborating with the interdisciplinary care team, PCP, member/member's representatives, and specialists. Utilize benefit structure and community-based resources to meet member's needs. Educate members on disease processes, preventative health, and quality of life interventions. Knowledgeable in and adhere to CMS/DOH regulations and policies. Requirements: 3-5 years clinical social experience in long term care, managed care, home care, geriatrics or hospice Prior experience in an interdisciplinary service delivery environment preferred. Working knowledge of Medicare and Medicaid regulations Prior experience in an interdisciplinary service delivery environment.
