Key Responsibilities

• Respond promptly to phone inquiries from members, providing timely assistance and support
• Conduct triage through phone consultations or in-home visits as required, ensuring that our members receive the care they need
• Collaborate effectively with on-call physicians, maintaining precise documentation and ensuring seamless communication

Required Skills and Qualifications

• Current Registered Nurse (RN) License in Pennsylvania
• Valid Driver's License
• A deep commitment to providing compassionate care to seniors
• Registered Nurse (RN) degree from an accredited institution
• Prior experience in home health or geriatrics is preferred
