As a Field Care Coordinator, RN, you'll play a vital role in coordinating care for members with complex medical and behavioral needs, promoting improved health outcomes and enhanced quality of life.

Key Responsibilities:

  • Conduct comprehensive needs assessments to identify medical, behavioral, and socioeconomic needs
  • Develop and implement personalized care plans, leveraging evidence-based guidelines and collaborative partnerships with internal care teams, providers, and community resources
  • Engage members through face-to-face and telephonic interactions, providing education, coaching, and support to promote self-management and lifestyle changes
  • Advocate for members and families, ensuring their needs and choices are fully represented and supported by the healthcare team
  • Utilize MS Office, including Word, Excel, and Outlook, to document and manage care plans, and maintain accurate records

Requirements:

  • Current, unrestricted independent licensure as a Registered Nurse
  • 3+ years of clinical experience, with 2+ years of relevant healthcare experience
  • 1+ year of experience with MS Office, including Word, Excel, and Outlook
  • Reliable transportation and ability to travel within assigned territory (up to 50% of the time)
  • Residence in Southwest or Southeast New Mexico, with flexibility to work remotely

Preferred Qualifications:

  • Master's degree or higher in a clinical field
  • Commission for Case Manager (CCM) certification
  • 3+ years of clinical experience as a Registered Nurse, with 1+ year of community case management experience
  • Background in Managed Care
