Employment Type: Employee Employment Classification: Regular Time Type: Full time Work Shift: 1st Shift (United States of America) Total Weekly Hours: 40 Additional Locations: Job Information: Summary: Serves as a single point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Virtua and in the community. Facilitates patient appointments, including those made with labs, diagnostic areas, and specialty physicians. Position Responsibilities:

  • Serves as an essential link between patients and all other care providers. Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies. Adheres to established productivity benchmarks.
  • Facilitates appointments for consults and support services within established service standards, including social workers, dietitians, physician, etc. Follows patients throughout the care continuum, including inpatient admissions, and collaborates with inpatient care management resources.
  • Coordinates multidisciplinary planning conferences, develops concise patient summaries for use by the care team, and documents recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
  • Develops, plans and presents patient education programs and tools.
  • Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff, etc.)
  • Works to collect data, track outcomes, and support strategic planning processes. Delivers quarterly report card to Virtua committees and other groups which demonstrate outcomes and performance improvement activities.

Position Qualifications Required / Experience Required: Experience in specialty required. Requires problem solving, decision making & critical thinking. Requires excellent leadership, organizational, written & verbal communication & excellent interpersonal skills. Must be able to work in a self-directed environment, with an ability to work with and lead teams. Excellent presentation skills. Ability to implement professional and community based education programs. Database or registry software experience preferred. Microsoft Office competency required. Required Education: BSN or related clinical diploma required. Training/Certifications/Licensure: Active NJ Licensed RN. Nursing Certification in specialty preferred Annual Salary: $77,405 - $123,574 The actual salary/rate will vary based on applicant’s experience as well as internal equity and alignment with market data.
