About the Role: Your Impact from Day One As a Virtual Physician, you will be a central figure in a multidisciplinary team focused on delivering high-quality primary care to patients in their homes. This full-time role is not just about treating acute illnesses; it's about building long-term relationships and proactively managing the health of our patient population, many of whom have chronic conditions or limited mobility. You will use a state-of-the-art telehealth platform and other digital tools to conduct virtual visits, provide diagnoses, and develop personalized care plans. Key Responsibilities Virtual Consultations: Conduct patient visits via video conferencing, phone, and secure messaging to diagnose and treat a wide range of acute and chronic conditions.

Comprehensive Care Planning: Develop and implement evidence-based, individualized care plans in collaboration with the patient and their family. This includes creating goals, monitoring progress, and making necessary adjustments.

EHR Management: Maintain accurate and timely documentation of all patient encounters, prescriptions, and care plans within our electronic health record (EHR) system.

Qualifications and Skills Medical Degree (MD/DO): A Doctor of Medicine or Doctor of Osteopathic Medicine degree from an accredited institution.

State Medical License: A current, unrestricted medical license in the state(s) of practice, with a willingness to obtain additional state licenses as required.

Telehealth Experience: Proven experience and comfort with conducting patient visits via telehealth platforms.
