About the Role

Aviator Health is actively recruiting Healthcare Navigators (Community Health Workers). With Aviator, you will work with Medicare patients throughout their healthcare journey, helping them overcome barriers to care and access the resources they need to thrive. We’re looking for a mission-driven, Texas CHW-certified or Licensed LPN/LVN professional who excels at building trusted relationships with patients and community partners. This role is ideal for someone with experience in patient advocacy, care coordination, or social services, and who is passionate about addressing social determinants of health in a hands-on, person-centered way. As a Navigator, you’ll play a key role in supporting patients remotely by guiding them through their care journey, addressing barriers, and connecting them to community resources. You’ll focus on building trusted relationships virtually, coordinating care with providers, conducting regular follow-ups, and ensuring patients receive warm hand-offs to the services they need. You’ll also help strengthen Aviator’s community presence by fostering connections with local organizations and sharing insights on resource availability. This is a W2 position with paid holidays, PTO and healthcare benefits available. The role is remote. Employees must reside in Texas and be CHW certified or a licensed LPN/LVN. What You’ll Do

  • Learn the Aviator systems, tools, technology, partners, and expectations, while also providing your unique expertise in every interaction.
  • Build strong, trusting relationships with Medicare patients, where listening and empathy are the foundation for every interaction.
  • Be able to identify and prioritize Medicare patients’ needs and assist them to maintain a streamlined care continuum.
  • Develop comprehensive patient care plans that holistically address social determinants of health, i.e. food resources, transportation access, and support at home.
  • Stay up to date on local resources that address social determinants of health (SDOH) and ensure patients are connected to the right services.
  • Support patients in addressing SDOH through warm hand-offs, weekly follow-ups, and hands-on support — including but not limited to navigating healthcare systems, applying for benefits, arranging transportation, accessing financial assistance, connecting with caregiver resources, and more.
  • Build relationships with hospital case managers, social workers, and physicians.
  • Help refine outreach playbooks, test what works in your region, and shape new workflows.

What You Bring to the Table

  • Active Texas Community Health Worker (CHW) certification (required) or current Texas LPN/LVN license.
  • 2+ years of experience in care coordination, patient advocacy, community health work, or healthcare navigation.
  • Deeply empathetic and understand the Social Determinants of Health alongside experience working with diverse patient populations.
  • Tech savvy: comfortable using digital platforms, electronic health records, secure messaging, and virtual meeting tools.
  • Access to a reliable computer, internet, and phone to perform remote work effectively.
  • A deep understanding of social determinants of health and experience working with diverse populations.
  • Strong communication skills with the ability to support patients and collaborate effectively with healthcare providers and community organizations.
  • Ability to manage multiple priorities, document clearly, and communicate proactively.
  • A local network of contacts at hospitals, SNFs, senior living communities, community-based organizations, or social service organizations (preferred).
  • Bilingual skills (preferred but not required).

Pay: $38,000.00 - $40,000.00 per year Benefits:
