[Remote] In-Home Dialysis Care Partner (1:1 Client) – Fluent Spanish Required

Herewith GmbH is seeking a compassionate and reliable Care Partner fluent in Spanish to support in-home dialysis for a patient transitioning from clinic to home care. The role involves providing safe and high-quality dialysis support, building trust with the patient, and ensuring a smooth care transition.
Responsibilities
- Assist with in-home dialysis treatment setup (NxStage or similar systems)
- Maintain a clean, organized, and safe treatment environment
- Support the patient in staying on schedule with dialysis treatments
- Provide companionship and emotional reassurance
- Assist with light Activities of Daily Living (ADLs) as needed
- Monitor patient safety and report concerns to the coordinating nurse
- Maintain simple documentation and shift notes
Skills
- Minimum 1 year of dialysis experience (home or clinic)
- Fluent in Spanish (required)
- Open to in-home dialysis care and completing provided training
- Calm, patient, and dependable—especially during care transitions
- Strong communication skills and ability to collaborate with a clinical team
- Must pass a Herewith background check
- Familiarity with home dialysis systems (NxStage a plus)
